Plan Document Services
New Plans
Consultation with Client to establish the priorities of the Plan.
Review of various design provisions to accomplish those goals.
Provide IRS approved Plan document and Summary Plan Description
Prepare all appropriate government regulatory forms
Description of Fiduciary requirements of the Plan
Identification of Fiduciaries and assignment of duties.
Customized Employee Booklets
Conduct employee meetings
Review plan operation with employers staff and outside vendors to ensure the efficient operation of the Plan
Existing Plans
Review existing documents and amendments of the Plan for compliance, including all plan design
specifications and trust requirements.
Review of Summary Plan Description and all other material or information provided to participants
and beneficiaries.
Review Investment Policy Statement and Investment Reports
Review of Fiduciary obligations with employer, staff and vendors.
Review of Plan in operation i.e., prior 5500, various compliance tests, assets, loans and distributions,
eligibility and vesting
Record Keeping Services
JPR Advisors provides all recordkeeping services required pursuant to the Plan and applicable law, including but not limited to the following:
Enter data from census.
Maintain records for all participants, including name, social security number, contribution amounts,
and other records as required.
Maintain records of participant accounts, deposits, withdrawals, investment gains/losses, investment
election percentages.
Provide participants with daily web access to their account information.
Provide participants and beneficiaries with participant statements (mailed directly to the address
last provided by Client to JPR within 30 Business Days after the end of each reporting cycle.
Prepare a summary of participant account activity for Client each reporting cycle.
Prepare a trust summary for Client each reporting cycle.
Plan Administration Services
JPR Advisors acts as a named fiduciary under ERISA and provides all administrative services required pursuant to the Plan and applicable law,
including but not limited to the following:
Determine participant plan eligibility.
Perform IRC Section 410(b) coverage testing.
Perform top heavy testing.
Perform preliminary and final ADP/ACP nondiscrimination testing, including any necessary refunds
or corrections to ensure compliance with applicable laws and regulations.
Perform annual IRC Section 415 testing and 402(g) analysis.
Allocate employer and employee contributions.
Prepare annual IRS Form 5500 series and applicable attachments.
Reconcile participant account balances to net asset totals on Trustee reports.
Prepare Summary Annual Report.
Prepare participant distribution paperwork.
Prepare IRS Form 1099-R and applicable State and Federal withholding forms after completion of a
distribution to a participant.
Prepare participant loan documentation, including loan application, amortized loan agreement,
acknowledgment and consent to loan, amortization schedule and Federal Truth in Lending statements.
Perform routine administrative consultation (up to two hours per reporting cycle).
